I hereby apply for admission to Yeshivas Ohev Shalom. I understand that enrollment at Yeshivas Ohev Shalom is dependent upon the maintenance of regular attendance and satisfactory work. Students are required to be familiar with and abide by the Yeshivas Ohev Shalom Code of Conduct and Handbook. Yeshivas Ohev Shalom reserves the right to require the withdrawal of any student at any time for disciplinary,academic, or other reasons it deems sufficient.
SCHOOL TRANSPORTATION AUTHORIZATION
I hereby authorize Yeshivas Ohev Shalom to provide transportation for my child to and from any school trip or function,including carpool to and from school.
OFF CAMPUS AUTHORIZATION
I hereby give permission for my child to walk off campus when permitted by the staff, and release Yeshivas Ohev Shalom of responsibility off school grounds.
